Western Springs Senior Center / District 101 COVID-19 Vaccine Clinic Waitlist Closed for Now

OUR WAITLIST HAS BEEN CLOSED FOR NOW DUE TO LARGE NUMBERS OF APPLICANTS. WE WILL INFORM THE COMMUNITY IF OUR WAITLIST OPENS UP AGAIN IN THE NEAR FUTURE. PLEASE KEEP CHECKING THIS PAGE FOR UPDATES.



Dr Ryan and the Cara Direct Care staff are delighted to be helping Western Springs Senior Center and Western Springs School District 101 with setting up a community vaccine clinic, with the aid of Jewel-Osco. Pharmacists will administer the vaccine and our community volunteers will help patients navigate the process.


Having local community vaccine drives is much more efficient than having our seniors drive to all corners of Chicagoland for their vaccine appointment. Please see further details below and consider offering your time as a volunteer. We are hoping that if this is a success, we can continue this program over the coming months for our community. Let’s work together to get our community protected from COVID 19!

WHAT?

Western Springs Senior Center/District 101 Covid 19 Vaccine Clinic - community vaccine program manned by volunteers and vaccines administered by Jewel-Osco pharmacists.


WHERE? To be determined - one of the local schools or village buildings

WHO? 1. BOTH DOSES ONLY: This clinic is for first and second dose recipients only. We cannot make appointment for your second dose at this time if you received first dose elsewhere. 2. PHASE 1A/1B ONLY: We cannot accept Phase 1C eligible at this time as Cook County is currently only serving Phase 1A/1B. Our focus is to serve the senior citizens in our town and as many senior citizens and qualified Phase 1A/1B individuals as possible.


CHECK IF YOU ARE CURRENTLY ELIGIBLE FOR COVID 19 VACCINE PROGRAM:


Please bring ID, insurance card (if any) and documents that verify your eligibility such as employment ID, letter for caregiver status from your physician etc.

Acceptable documentation can include, among others, personal identification (State ID or driver’s license, US or foreign passport, Consular ID, Temporary Visitor Driver License), or employee identification (work identification or badge, paycheck stub, CityKey card, or letter from employer or physician (if caregiver status).

3. DO NOT WAIT IF YOU GET A VACCINE APPT ELSEWHERE! Take the first vaccine appointment you are offered. Please delete your waitlist list spot if you receive the vaccine elsewhere. 4. ALL POPULATIONS IN ILLINOIS CAN RECEIVE THE VACCINE REGARDLESS OF INSURANCE COVERAGE OR IMMIGRATION STATUS, INCLUDING INDIVIDUALS WHO ARE UNDOCUMENTED.


WHEN? The date(s) for the first and second vaccine appointments have not yet been established, but if you would like to be put on the notification list, please complete this form. Once the dates have been finalized, volunteers will be reaching out to individuals to schedule appointments. We hope to schedule Dose 1 sometime in early-mid March 2021. Second dose appointments will be scheduled prior to leaving your first dose appointment.


HOW?

HELP YOUR LOCAL SENIORS! If you know of a senior citizen who is in need of a vaccine, please pass along this information. You may also sign up a senior citizen and serve as their contact person as we know some people do not have internet access or email addresses.


WHAT ELSE?


VOLUNTEER: Would you like to help facilitate vaccine distribution and administration? For now we are taking non-clinical volunteers only.
