Pricing

CHILDREN:

 

0-5 years:      $60/month with paying adult family member

​

6–18 years:   $50/month with paying adult family member

 

ADULTS:

 

18-44 years:   $80/month

​

45–64 years:  $105/month

​

65+ years:     $130/month

​

EMPLOYERS:

 

5 or more employees: $80 per employee/month (paid directly by employer), no enrollment fee

​

0-5 employees: Age-based rate, no discounts

​

TERMS AND CONDITIONS:

A registration fee of $100 per person or $200 per family will be added to all first month payments. Re-enrollment fee after cancellation will be $300 unless membership reinstated within 3 months with payment for months missed. Patients are billed once monthly and may discontinue their membership at any time with 30 days notice and signed cancellation form.
